Survival Swim with Molly Brooks
By registering your child for survival swim lessons, you (the parent or legal guardian) agree to the following Terms & Conditions:
1. Assumption of Risk & Liability Waiver
I understand that participation in survival swim lessons involves inherent risks, including but not limited to slips, falls, water inhalation, and other potential water-related injuries. I voluntarily assume all risks associated with participation and agree not to hold Molly Brooks, Survival Swim with Molly Brooks, any affiliates, staff, or facility owners liable for any injury, illness, accident, or loss that occurs during or as a result of lessons.
2. Medical Disclosure & Emergency Care
I certify that my child is physically able to participate in survival swim lessons. I will inform Molly Brooks of any medical conditions, behavioral concerns, allergies, or developmental needs prior to the first lesson. In the event of a medical emergency, I authorize Molly Brooks to seek emergency care and understand that I am responsible for any associated costs.
3. Parent/Guardian Presence & Conduct
I understand that a parent or legal guardian must remain on-site during all lessons unless given express permission otherwise. I agree to follow all facility and program rules and to support a calm, quiet environment for lessons, as distraction can compromise safety and learning outcomes.
4. Lesson Structure & Expectations
I acknowledge that survival swim lessons are customized, one-on-one sessions designed to teach critical aquatic survival skills. I understand that progress varies by child and is influenced by consistency, behavior, temperament, and developmental readiness. I agree to support the program by avoiding interference and allowing the instructor to guide each session without disruption.
5. Payment & Refund Policy
Payment is due in full prior to the start of the first lesson. Missed lessons are non-refundable. In cases of illness or emergency, a makeup lesson may be offered at the discretion of the instructor, pending availability. No refunds are issued for no-shows or voluntary withdrawal once lessons have begun.
6. Cancellation & Rescheduling
A minimum of 24 hours' notice is required to cancel or reschedule a lesson. Cancellations with less than 24 hours' notice may result in forfeiture of that lesson. The instructor reserves the right to reschedule lessons due to illness, emergencies, inclement weather, or unsafe pool conditions. Make-up lessons will be offered when possible.
7. Weather & Pool Conditions
Outdoor lessons may be canceled due to thunder, lightning, or unsafe pool conditions. These lessons will be rescheduled at the instructor’s earliest availability. No refunds will be given due to weather-related delays.
8. Flotation Devices & Swim Aids
I understand that survival swim instruction does not involve the use of recreational flotation devices (such as puddle jumpers, floaties, or arm floaties). These interfere with muscle memory and proper water movement. U.S. Coast Guard-approved life jackets may be used only when appropriate and approved by the instructor.
9. Photo & Video Release
I grant permission for Molly Brooks and Survival Swim with Molly Brooks to take photos or videos during lessons for educational, documentation, or promotional purposes unless I provide written notice opting out.
10. Communication & Questions
I agree to communicate any questions or concerns respectfully, outside of lesson time, via the designated communication channel (email, phone, or messaging platform). I understand that consistent communication and mutual respect are essential for my child’s success and safety.
Agreement Acknowledgment
By registering for survival swim lessons, I acknowledge that I have read, understood, and agree to abide by the above Terms & Conditions. I certify that I am the parent or legal guardian of the enrolled child and am authorized to accept these terms on their behalf.
